Your homepage will link to this single page customer-survey form. This form will contain your questions with associated answers and comments (up to 40 fields) entered by Internet users (new or existing customers). Their responses will be transferred via Internet email to one or more persons (specified by you at time of development), and logged to an ASCII text file transferred via email FTP to your office periodically or upon request. Up to 4 MB of report information may be transferred monthly.
